Do you accept insurance?
I am eligible as an out-of-network provider for most insurance plans and will work with you to make sure you maximize your out-of-network coverage.
Below are some questions you may want to ask your insurance company to understand how much of the fee will be covered.
• Does my plan include out-of-network mental health benefits and do I need preauthorization to access those benefits?
• What is the deductible under my plan and has part or all of it been satisfied?
• What percentage of the cost of treatment will be my responsibility once my deductible has been met?
• Is there a limit to the number of sessions my plan will cover?
I will supply an invoice at the end of each month in order for you to file a claim with your carrier.
What is your fee?
My fee is $265 for individuals and $325 for couples therapy.
I reserve some openings for reduced-fee sessions. If you are interested in a reduced fee arrangement, please let me know in advance.
Will what we talk about remain confidential?
All material discussed in our sessions will remain confidential, subject to certain legal requirements to disclose in the case of the risk of suicide or harm to others or the suspicion of child sexual abuse. If you use insurance, you should be aware that certain confidential information might be demanded by the insurance company as a condition of reimbursement. All such disclosures will be discussed with you in advance. I am HIPAA compliant.
What is your cancellation policy?
I request that 24 hour notice be provided for any cancellations or rescheduling. Less than 24 hour notice will result in the charge of the full session rate.
What forms of payment do you accept?
I accept Zelle, Venmo, check and credit card payments, preferably at the time of service unless otherwise agreed upon.
How do I get there?
My office is located on 28th Street and 5th Avenue in the Flatiron District of Manhattan. The N and R trains are one block away at 28th and Broadway and the 6 train is two blocks away at 28th and Park Avenue.
